2026-27 Transportation Forms
Dear Parent(s)/Guardian(s),
Please review the options below. Selecting the correct request type helps us process your request accurately and as quickly as possible.
For Newly Registered Students
Once your student's registration is complete, the school's building secretary will contact you with your child's bus transportation details.
Newly registered students do not need to submit a separate transportation request. Transportation needs and address information are collected during the student registration process. If you need to register your child for school, please visit the Student Registration page.
For Currently Enrolled Students
Please select the option below that best matches your current transportation or address update needs.
Use this option only if your child's home/residential address has changed. Please provide your new address and upload proof of residency.
If your home address has not changed and you only need to change your child's pickup or drop-off location, please use Option 2.
Use this option for pickup or drop-off changes when your home address has not changed, including requests for an alternate address, new transportation arrangements, or special/emergency transportation needs. Please provide the requested transportation details.
Use this option if your child will not be using district-provided transportation.
This request is not permanent. If your transportation needs change, simply complete the New/Alternate/Emergency/Special Transportation Request (Option 2) to request district-provided transportation.
This request is ONLY for students who are residents of the Oneida City School District AND attend St. Patrick's Elementary School or Holy Cross Academy.
Complete this form to request district-provided transportation. Please provide all required transportation information.
Processing Time
Please allow up to three (3) business days for the Transportation Department to process your request. Once your request has been reviewed, the Transportation Department will notify you of any updates.
If you do not receive a phone call by the end of the third business day, please contact the Transportation Department at 315-363-5470.

Important: New York State Education Department regulations require transportation requests for the following school year to be submitted by April 1. Families who establish residency in the Oneida City School District after April 1 must submit a transportation request within 30 days of establishing residency to be eligible for transportation.
